Some patients do not want larger breasts but feel that age, gravity, weight loss, or child bearing have caused their breast to sag, have less volume, and be less firm. These women are often best treated with a breast lift.
If the nipples are sagging below the level of the natural fold under the breast, a breast lift is usually needed. In some women only the breast skin will sag below the fold, but the nipple stays above the fold. This often does not require a breast lift and can corrected with just breast implants.
The breast lift incisions are made around the areola (the dark skin around the nipple) from the bottom edge of the areola down to the crease under the breast and in the natural fold under the breast. A breast lift removes the excess skin; the breast is lifted and made firmer. The areola may also be made smaller. Patients are pleased with the restoration of a youthful appearing breast.
Following a breast lift, there will be no stitches to remove and usually no drains. You may gradually resume your normal activities during the first week after surgery.
Short Scar Breast Lift
An innovation in breast lift surgery is a technique performed by Dr. Hackney that limits the length of the scars. If the nipple only needs to be lifted a small amount, a circum-areolar breast lift may be the best option for you. This sometimes called a “purse string” breast lift. This technique limits the scar to around the edge of the dark skin around the nipple. The dark skin around the nipple is called the areola. If the nipple needs to be moved higher or the breast made smaller, in addition to the incision around the areola, a vertical incision is made down to the natural fold under the breast. The combination of the scar around the areola and the vertical scar below the areola is called a short scar breast lift.
The purse string breast lift is especially useful for a condition referred to as a tubular or tuberous breast. Young women who have not had children and come to our office for a breast lift, often have this. For more about tubular breast and a breast lift, please see the Breast Implants page, How to Obtain Natural Appearing Breasts.
Breast Lift and Implants Controversy
Some patients with small breast and nipples that are positioned below the level of the breast fold will need a breast lift and implants. However be aware there is disagreement between experienced plastic surgeons regarding how to do this. Some surgeons will do both the breast lift and implants at the same time and some surgeons recommend doing two separate operations. Among the surgeons who do them separately, some will do the breast lift first and the implants a few months later and other surgeons will do the implants first and the breast lift second.
